Instructions:

1. Enter your account number and account name.
2. You may click on the category of the supply you would like to order and be linked to that category of items, or you may simply scroll through the order form.
3. Enter the quantity of items needed in the appropriate box.
4. Note: At the bottom of the page you will find a comment box. Please use this space for questions and/or to request an item that you do not find on the order form.
5. Once you have filled in ALL quantities for ALL desired items simply click on the Order button at the very bottom of the page.

Please remember that we can only supply your office or facility with those items that will specifically be used to collect specimens which you will be returning to MuirLab. For more information about MuirLab's transport devices and items, review our Physician Resources page.
